The Management of Malocclusion in Adult Patient with Clear Aligners: Key Points

The aim of the present lecture is to highlight the key points to treat adult patients by means of clear aligners. Increasing demand of aesthetic treatment, allowed for a progressive improvement of technology as well as of doctor's expertise. To manage all type of malocclusion, it is necessary to understand the biomechanics challenges of clear aligners such the anchorage management, the amount of dental movement and the staging of treatment phases. First of all anchorage, defined as the resistance used to overcome the reaction of an applied force, is a key factor to perform a successful treatment. Clear aligners, as many other therapeutical systems, use teeth as anchorage to resist to unwanted movement. The combined root surface area of the anchor teeth should be two times that of the teeth to be moved and therefore a proper planning of anchorage should be performed for the different treatment phases. Finally, the amount of dental movement and staging affect clinical effectiveness and treatment duration. In order to successfully deliver orthodontic therapy, possibilities and limitations of each treatment modality should be carefully assessed.
